Creates a page for 'Report a problem'. Missed collections, containers not delivered, spillages, containers not returned will be reported on this page instead of on the bin days page. The bin day page will have a 'Report a problem' link under each service which will direct to the new page, instead of individual links for each type of problem. Escalations of reports is included and will be the location of disputes as well when they are implemented. mysociety/societyworks#5502

Change to having one page with all report a problem options, rather than individual problems that can can be reported listed under the service on the bin days page.
