Version bump and CHANGELOG heading for #66 and #67. No source changes; 324 tests pass.
The live wallet is on v0.12.0, so this deploy also picks up 0.13.0 and 0.14.0 - the
m/139'derivation migration among them. That migration keeps separate counters per ladder and restores from both, and a wallet that predates it never mints on the new one, so the two cannot collide.
