Summary

  • Cut code-review token use the same way as implement ingest: index the diff (--stat / --name-status + hunk Reads), cache the reviewer profile, and stop stuffing AGENTS/guidelines.md/raw patches into the reviewer.
  • /continue re-review gets the latest round plus a short already-decided list, not the full review history.
  • Bump code-review 0.2.1 → 0.3.0. Stacked on Reduce implement ingest tokens; keep plan quality generic #109.

Test plan

  • Confirm this PR's diff is code-review only (base cheaper-implement-ingest)
  • /code-review:start on a small uncommitted change: no full-diff dump in the reviewer prompt, .artifacts/code-review/_reviewer-profile.md written, findings still file+line cited
  • /continue does not reload every prior code-review-*.md / review-response-*.md

Summary

  • Updates the code-review workflow to index diffs with --stat and --name-status, record status-aware hashes, and cap contextual hunk reads at 20 files.
  • Excludes generated, vendor, lock, raw patch, AGENTS.md, CLAUDE.md, and guidelines.md content from reviewer prompts.
  • Adds cached reviewer profiles, change summaries, diff indexes, review templates, stable finding IDs, and finding-reference validation.
  • Updates re-reviews to use the latest review, aggregated decisions, changed-file hash checks, and limited context loading.
  • Updates /start and /continue to resolve phase overrides and avoid redundant controller reads.
  • Adds handling for untracked-only workspaces, bounded untracked-file reads, unattended-mode visibility, structured decision questions, and stat-only coverage reporting.
  • Updates controller guidance for reviewer context, profile caching, convention-file handling, session reuse, and one-time artifact writing.
  • Updates cleanup guidance to list all round-specific artifacts.
  • Bumps the code-review skill version to 0.4.0.
  • Changes do not modify _shared/ resources or cross-workflow conventions.

Walkthrough

Changes

The code-review workflow now resolves phases through shared recipes, caches reviewer profiles, indexes diffs, limits hunk reads, validates finding paths, supports independent re-review, and uses structured templates.

Code review workflow

Layer / File(s) Summary
Command routing and reviewer context
code-review/SKILL.md, code-review/commands/*, code-review/skills/controller.md
Commands use session-aware controller loading and shared phase-override resolution. Reviewer context excludes raw patches and unrelated guidance files.
Cached profile and indexed start review
code-review/skills/start.md
The start phase caches convention data, captures diff indexes and hashes, limits hunk reads, validates finding paths, and writes review artifacts once.
Independent re-review and artifact cleanup
code-review/skills/continue.md, code-review/skills/clean.md
Re-review uses an independent reviewer, current diff indexes, bounded inputs, changed-file hash checks, and validated citations. Cleanup includes all round-specific artifacts.
Workflow artifacts and documentation
code-review/README.md, code-review/templates/*
Documentation describes the cached profile and indexed artifact layout. Templates define reviewer profiles, change summaries, findings, questions, and verdicts.

Estimated code review effort: 3 (Moderate) | ~25 minutes

Merge Risk: 🟡 Moderate · up to d56f9

This PR changes how review diffs, reviewer context, and re-review decisions are stored and consumed. The current implementation can misassociate findings between rounds, write required artifacts to the wrong location, skip deleted-file content, and pass ambiguous diff-index inputs, so it is not merge-ready until these bounded correctness and integration issues are addressed.

Explanation

No functions found in the changed files to evaluate docstring coverage. Skipping docstring coverage check. Docstring coverage is scoped to functions touched by this diff. Analyzed 0 functions across 0 files. (2 skipped: 2 unsupported.)

Full details: Ai-Attribution

Explanation

AI use is explicit in the pull-request commits. All 7 commits in f7f8c6d..HEAD contain the accepted Made-with: Cursor trailer. No Co-Authored-By trailer appears in the pull-request commits. The attribution check passes.

Full details: No-Absolute-Paths-In-Skills

Explanation

PASS — The pull request introduces no prohibited absolute filesystem path in the scoped workflow Markdown files. The diff against main changes only code-review skill, command, and manifest files; added path references are relative (for example, ../skills/... and ../../_shared/...). The existing /dev/null redirections in clean.md and continue.md are unchanged from main and are not pull-request-caused.

Full details: Command-Colon-Notation

Explanation

All command files have YAML frontmatter with a name field in {workflow}:{phase} form, and every prefix matches the parent workflow directory. The pull request changes code-review/commands/start.md and code-review/commands/continue.md; both retain valid names: code-review:start and code-review:continue. The full audit found zero invalid command files.

Full details: No-Orphaned-References

Explanation

PASS. All changed Markdown files are under code-review/. The changed source references resolve: controller, guidelines, phase-override recipe, shared review protocol, and all three templates exist. The controller references start.md, continue.md, and clean.md, and SKILL.md dispatches command files through commands/{command}.md; every command and skill file is reachable. Generated artifact names and project-discovery inputs are runtime/output paths, not dangling workflow-file references.

Full details: No-Content-Duplication

Explanation

PASS — code-review/SKILL.md and code-review/skills/controller.md changed; guidelines.md did not. The SKILL.md change only bumps the version. Comparison of all three present files found no identical multi-line blocks, paragraphs, or normalized paragraphs. Similar guidance remains role-specific rather than verbatim duplication.

Full details: Step-Sequencing

Explanation

code-review/skills/continue.md adds #### 7a: Validate finding references between ### Step 7 and ### Step 8. This is an unconditional workflow stage, not a conditional branch. It is a new main step hidden as a sub-step, which violates the sequencing rule. The other changed skill files have sequential numbered main steps; start.md's existing 7a/7b headings were already present in main and are not introduced by this pull request.

Review

The architectural changes here are well-designed and mirror the same proven patterns from PR 109: diff indexing instead of dumping, reviewer profile caching, template extraction, lighter re-review loading (latest round + already-decided list instead of full history), and clean separation of reviewer vs. implementor guidance (review-protocol.md instead of guidelines.md). The new 7a: Validate finding references step for re-reviews closes a real gap. These are the changes that deliver the token savings.

However, the PR also compresses instruction prose throughout start.md and continue.md. The instruction compression saves a small fraction of the tokens while carrying disproportionate quality risk — it doesn't meet the bar of saving tokens without degrading quality. Specific items that should be restored to their original detail:

start.md:

  1. Step 4 (discovery sources on cache miss): The old version listed specific linting config examples (.eslintrc, .golangci.yml, pyproject.toml) and explicitly called out "test configuration — test framework, directory structure, patterns." These help the LLM know where to look.

  2. Step 5 (relevance determination): The old version had explicit include criteria (source code, tests, supporting config changes, related docs), exclude criteria (scratch notes, personal drafts, unrelated modifications that predate the current work), and "when excluding, note it in the change summary with a brief reason." The template has the Excluded Files table, but the judgment criteria for populating it are compressed away.

  3. Step 6 (reviewer perspective): Two things dropped. First: "Read the full files around changed sections to understand context: does a new function duplicate existing functionality? Is the error handling pattern consistent with the rest of the file? Does the change interact correctly with surrounding code?" This is exactly the guidance that makes the hunk-read approach work well — without it, the reviewer may treat each ~80-line slice in isolation. Second: "evaluate the code as if you did not write it." Both are sentences, not paragraphs.

  4. Step 7a (hallucination warning): The old version explicitly said "AI reviewers hallucinate file paths and line numbers" as the reason for validation. That framing makes the LLM take the validation step more seriously than the compressed version.

  5. Step 7b (value assessment): The old version had: "Do not dismiss findings just because they are LOW or nit-level. A well-placed rename or a small clarity improvement can add real value. Equally, do not accept findings reflexively just because the reviewer flagged them." This prevents the two most common failure modes in LLM-driven review assessment. The new version defers to review-protocol.md with no inline reinforcement.

  6. Step 9 (user prompt): The explicit menu of options ("accept all, override specific, add guidance, correct question answers") tells the user what they can do. Also, the old decision table had four example rows including LOW findings being both accepted and rejected — this demonstrated that LOW findings belong in the table.

  7. Unattended mode rationale: The old version explained why the table is still presented: "the user can interrupt at any time, and the table gives them the information to decide whether to do so. Do not skip the table because the mode is unattended; unattended means the user delegated decisions, not visibility." Without the rationale, the LLM may decide the table is unnecessary in unattended mode.

continue.md:

  1. Re-review scope: The old version had "Focus on the current state of the diff, not just the delta from last round." The new version covers verifying accepted findings and checking for new issues, but the explicit instruction to evaluate the full current state is gone.

Other items (same as PR 109):

  • .meta.json schema: Same concern as PR 109. Add a concrete 5-line example so LLM sessions produce consistent cache files. If this is fixed in PR 109, it carries through here.

  • Hunk-read cap: If the ≤20 cap is hit, the reviewer should note which files got only stat-level coverage so the user knows what wasn't deeply reviewed.

Summary: The architecture is right — keep all of it. The instruction compression is a separate concern that saves a small fraction of the tokens while making the LLM less reliable on edge cases and judgment calls. Restoring the compressed detail keeps nearly all of the token savings while preserving review quality.

Flag any absolute filesystem path in markdown files within workflow directories (*/SKILL.md, /skills/.md, /commands/.md, */guidelines.md). Paths like /home/, /Users/, /tmp/, /var/, /opt/ are prohibited because workflows are installed via symlink and must use relative paths only. Paths inside fenced code blocks that are clearly examples (containing "example", "e.g.", or placeholder usernames like /home/user/) are exempt.

When any of SKILL.md, guidelines.md, or controller.md in a workflow is changed, compare it against whichever of the other two files are present and check for verbatim duplication of multi-line instruction blocks or paragraphs. Each has a distinct role: SKILL.md is the thin entry point, guidelines.md holds principles/limits/safety/quality/escalation, controller.md manages phase dispatch. Phase names and brief one-line descriptions appearing in multiple files is EXPECTED (cross-referencing, not duplication) — only flag substantial blocks of identical prose or step-by-step instructions that are copied between files.

Flag sub-steps that are actually new main steps inserted to avoid renumbering — those should be promoted to full steps with the sequence renumbered.

When workflows invoke commands that can affect shared systems, follow the documented approval and safety gates: verify git status before destructive Git operations, confirm branch and base before PR/MR pushes, require explicit approval for Jira writes, and run Vale before applying documentation changes.

conditional branches off a parent step — never as a way to
insert a new main step without renumbering
  • Internal cross-references (e.g., "see Step 4") must point to
    correct step numbers
  • No step should depend on output from a later step
  • Synthesis tasks (summarization, assessment, verdict) must NOT
be buried after heavy per-item processing — they degrade in
    long contexts
  • controller.md must reference sibling skills as phase-name.md
(not skills/phase-name.md) — relative to its own directory
  • Skills referencing _shared/ resources must use the correct
    relative path depth (e.g., ../../_shared/recipes/self-review-gate.md

  • Failure modes must be documented: what to do when prerequisites
are missing, when zero results are returned, when tools are
    unavailable
  • Escalation criteria must be clear: when to stop and ask the user
  • Instructions must be unambiguous — an AI agent reading
top-to-bottom should produce correct output on the first try

Learning: In markdown files under the repo’s skill/command areas (e.g., `skills/**` and `commands/**`), any references to other files on disk (like links/includes pointing to other skill/command markdown such as `../skills/controller.md` or `commands/*.md`) must use relative paths—never absolute paths (no leading `/` or fully-qualified filesystem paths). This ensures the references remain symlink-safe and resolve correctly at runtime. Do not apply this rule to human-facing prose docs like `README.md`/`CONTRIBUTING.md`; when those documents intentionally distinguish user-level vs project-level install locations, keep the absolute user-level paths (e.g., `~/.cursor/commands/`) as written so the distinction is clear.

1) For cross-document markdown links (e.g., links to other .md files like ../skills/controller.md or ../../templates/design.md), use paths relative to the current markdown file’s location so links work under symlinks.
2) For runtime artifact paths used as prose instructions to the AI agent (e.g., .artifacts/design/{issue-number}/publish-metadata.json or .artifacts/prd/config.json), keep them repo-root-relative (start with .artifacts/). Do not convert these artifact paths to be relative to the skill file directory (e.g., don’t rewrite to ../../.artifacts/...), because the AI resolves them from the repo root.

Restored the instruction prose you called out, without dumping git diff HEAD again.

start.md

  1. Cache-miss discovery examples (.eslintrc, .golangci.yml, pyproject.toml, test config) plus a concrete .meta.json schema with skill_version
  2. Include/exclude criteria, with exclusions and --stat-only files listed in the change summary
  3. Hunk context questions and “evaluate as if you did not write it”; if the 20-hunk cap is hit, list the files that got stat-only coverage
  4. “AI reviewers hallucinate file paths and line numbers” — verify the location exists
  5. Do not dismiss LOWs; do not accept reflexively
  6. User prompt menu and LOW rows in the decision-table example
  7. Unattended still shows the table — delegated decisions, not visibility

continue.md
8. Re-review the full current uncommitted change, not only last-round delta
9. Persist diff-index-{NNN}.json (name-status + hashes); re-check rejected findings only if that file’s hash changed this round

Also took the non-conflicting CodeRabbit notes: exactly one writer of the review file, Finding {N} with a repeat instruction, stop if the index commands fail, copy in-session AGENTS into the profile without re-Read. Shared cache stays at .artifacts/code-review/ so it survives cleanup.

Thanks for the review.

Invalidate the cache when every discovery input matches.

The cache miss reads linting and test configuration, but the cache hit checks only the listed convention files, CI filenames, and skill_version. A change to pyproject.toml, another lint configuration, or CI workflow contents can reuse stale reviewer guidance and quality gates. Record and compare every discovery input used by the miss path.

Addressed the follow-up CodeRabbit comments:

  • Empty git diff HEAD is not a failure when the workspace has only untracked files; stop only on command errors or an empty combined index
  • Untracked files get a first ~80-line Read plus one continuation; those count toward the hunk cap
  • Already-decided list aggregates every decisions-*.json; unattended will not accept a prior reject unless that file’s hash changed
  • Coverage tables instruct repeating each row; review template uses {round} vs {finding_number}
  • README states the reviewer cache is at .artifacts/code-review/, not under {branch}/
  • /clean lists whatever ls returned, including every diff-index-{NNN}.json
  • Skill references use ../guidelines.md and ../../_shared/review-protocol.md

Left 7a as a labeled sub-step (same as start). Did not expand .meta.json past the ingest-shaped sources plus skill_version.

Build a short **already decided** list from **every**
`decisions-*.json` in the branch artifact directory, not only the latest
round. Compact entries: file, location, accept/reject. Key by file +
location. A finding rejected in round 1 stays settled in later rounds.
Unattended mode must not accept a previously rejected finding unless
that file's hash changed this round and the new review re-raised it.

Key decisions by a unique finding identity.

continue.md keys decisions by file + location, but the shared finding contract does not require that pair to be unique. Two findings at the same location can therefore collapse into one decision, causing unattended mode to reject a different finding. Use a stable finding ID or fingerprint, or explicitly define and validate a one-finding-per-location invariant.

The Validate Versions check failed because main already shipped code-review 0.3.0 (the shared-rules MINOR bump). This PR is now 0.4.0.

Also:

  • Restored Andy’s original 7a wording (hallucinated paths/lines, discard findings that cite unchanged files or missing locations, note discards and why internally). I had left the 7a label for Andy and kept a compressed body; the body is what he asked to restore.
  • Settled decisions are keyed by file + location + title, so two findings at the same location do not collapse.
  • Question blocks in the review template are explicitly repeatable.

The restorations look good — all 8 items are addressed well. Three minor compressions remain in start.md Step 9 that should be restored for the same reason as the others: they don't contribute meaningful token savings and they make the LLM's behavior less predictable.

1. Unattended escalation detail

Old: "stop and escalate to the user — present the finding and explain the disagreement. Wait for the user to decide."
New: "stop and escalate."

"Stop and escalate" is ambiguous about what the LLM should actually do. Restore the explicit instruction to present the finding and explain the disagreement.

2. Questions presentation format

Old: Had a **Q1:** {question} / **Proposed answer:** {answer} format example for presenting reviewer questions with the implementor's proposed answers.
New: "present them below the table with proposed answers."

The reviewer writes questions using the template format, but the parent presenting those questions with proposed answers lost its format example. Restore the example so presentation is consistent across runs.

3. Decisions guidance example

Old: Three JSON entries including {"finding": 3, "decision": "accept", "guidance": "use approach X"}.
New: Two entries, no non-null guidance example.

The guidance field is visible with a null value, but there's no example of what a populated value looks like. Restore the third entry so the LLM knows how to record user-provided guidance.

Stop dumping full patches and convention docs into the reviewer;
pass a stat/name-status index, hunk neighborhoods, and a cached
profile. Continue only sends the latest round. Bump to 0.3.0.

Made-with: Cursor
MD004 treated a plus as a list marker; MD058 requires blank lines
around the change-summary tables.

Made-with: Cursor
Put the discovery, include/exclude, hunk-context, citation, and
decision-table prose back. Keep the diff as an index.

Made-with: Cursor
Stop treating untracked-only workspaces as index failures, read new
files in bounded chunks, and carry settled decisions across rounds.

Made-with: Cursor
Main already shipped 0.3.0. Key settled decisions by file, location,
and title so two findings at one location do not collapse.

Made-with: Cursor
Unattended CRITICAL disagreements now present the finding and wait.
Question presentation and a non-null guidance JSON entry are back.

Restored the three remaining Step 9 compressions:

  1. Unattended CRITICAL disagreement: present the finding, explain the disagreement, and wait for the user
  2. Reviewer questions use the Q1/Proposed answer format example
  3. Decisions JSON includes a third entry with "guidance": "use approach X"

Also rebased onto current main. Workflow stays 0.4.0.

Include every profile input in cache validation.

The cache miss reads linting and test configuration, and code-review/templates/00-reviewer-profile.md stores those values under Quality Gates. The cache-hit predicate checks only selected convention files and CI filenames. If a lint or test configuration changes, an old reviewer profile remains valid. Record and compare every discovery input.

Give each finding a stable id, hash deletes and renames by status,
pass the review template to the subagent, and drop context-only cites.

Addressed the four new CodeRabbit comments:

  • Findings carry a stable id ({file}|{location}|{title}, with #{N} on collision). Already-decided and unattended both match on id only — a prior reject stays rejected.
  • Diff-index hashes are status-aware: working tree for M/A/U, HEAD blob for D, new path + from for R.
  • Subagent inputs now include ../templates/code-review.md.
  • 7a requires the cited file on the name-status/untracked index and the location to overlap the change, not a neighboring context Read.

Did not expand .meta.json past the ingest-shaped sources plus skill_version.

Use one branch-scoped path for all round artifacts. The two phases write indexes under {branch}/..., while the documented artifact root is .artifacts/code-review/{branch}/.

  • code-review/skills/start.md#L168-L171: write 01-change-summary.md and diff-index-001.json under .artifacts/code-review/{branch}/.
  • code-review/skills/continue.md#L259-L265: write diff-index-{NNN}.json under the same directory.

The artifact layout is defined in code-review/README.md. Based on learnings: runtime artifact paths in skill instructions must remain repository-root-relative under .artifacts/.

Handle deleted files explicitly in both read procedures. Status D paths have no working-tree file, so the current bounded-read rules cannot review their removed content.

  • code-review/skills/start.md#L201-L212: read deleted content from the HEAD blob with a bounded command such as git show HEAD:{path}.
  • code-review/skills/continue.md#L276-L288: apply the same deleted-path rule during re-review.

Name the exact diff-index input passed to each subagent. The current wording can be read as raw command output instead of the status-aware JSON artifact.

  • code-review/skills/start.md#L218-L223: name .artifacts/code-review/{branch}/diff-index-001.json.
  • code-review/skills/continue.md#L293-L300: name .artifacts/code-review/{branch}/diff-index-{NNN}.json.

As per path instructions, workflow instructions must be unambiguous.

Repo: flightctl/ai-workflows PR: 47

Learning: In flightctl/ai-workflows skill markdown files, treat path references as two categories:

Once the user states decisions, persist them to
`.artifacts/code-review/{branch}/decisions-{NNN}.json`. Set `id` to
`{file}|{location}|{title}`; if two findings in this round would share
that string, append `#{finding_number}`.

Use a stable finding identity across review rounds. The current ID includes a mutable title and a round-local collision suffix.

  • code-review/skills/start.md#L337-L349: generate and persist a stable ID with each finding.
  • code-review/skills/continue.md#L267-L274: aggregate decisions by that stable ID.
  • code-review/skills/continue.md#L481-L487: apply the same stable ID when blocking previously rejected findings.

This repeats the earlier stable-identity feedback because the current implementation still uses the mutable composite key.

## Summary
- Cut code-review token use the same way as implement ingest: index the
diff (`--stat` / `--name-status` + hunk Reads), cache the reviewer
profile, and stop stuffing AGENTS/`guidelines.md`/raw patches into the
reviewer.
- `/continue` re-review gets the latest round plus a short
already-decided list, not the full review history.
- Bump code-review **0.2.1 → 0.3.0**. Stacked on flightctl#109.

## Test plan
- [ ] Confirm this PR's diff is code-review only (base
`cheaper-implement-ingest`)
- [ ] `/code-review:start` on a small uncommitted change: no full-diff
dump in the reviewer prompt,
`.artifacts/code-review/_reviewer-profile.md` written, findings still
file+line cited
- [ ] `/continue` does not reload every prior `code-review-*.md` /
`review-response-*.md`
