Purpose
The
MISSING_PARAMETERfailure reason was added toWorkflowFailureReasonbut never wired up — it had no error message, no entry in the failure reason message map, and no handler inModalOrchestrator, so it would silently fall through to the generic error. This PR completes the implementation.Summary
