feat(review): add "Refresh" action to re-sync review message with GitLab

Adds a new overflow menu entry on review messages that re-fetches all MR data from GitLab (state, approvals, pipeline, discussions) and updates the Slack message in place. Useful when Homer misses a webhook and the message is out of sync. A permalink to the refreshed message is included in the ephemeral confirmation sent to the user.
