Reproduction steps

Been seeing this starting with v0.230.1. Not sure how to reproduce other than sending a prompt that would call a tool.

Creating this issue as a placeholder for others or myself later to help fill in the blanks.

Nothing in the log from now (~14:32), when I saw another set of three notifications.

Hey @tredondo -- jw: do you have notify_when_agent_waiting set to all_screens?

@yeskunall: yes. I only have one screen, but three virtual desktops in KDE. The setting has been the same for many versions of Zed, but I've only seen this triplicate behavior recently (and intermittently).
