feat(do): Leverage ctx.exports for automatic DO WorkerEntrypoint binding

Enhanced Worker Loader integration to use Cloudflare's `enable_ctx_exports` flag for automatic loopback bindings:

1. **ctx.exports Integration** - Pass ExecutionContext to executeCode() to access ctx.exports.DO
2. **Automatic Loopback Binding** - Prefer ctx.exports.DO over explicit env.DO binding
